Don't go to a shop for stretching. You should stretch your own ears so you can stop if it hurts, there's too much resistance, etc. By "you don't need a piercing to start with" it seems like they mean they're willing to pierce at a larger-than-normal gauge and then immediately stretch it up a size. Or they might just mean large-gauge piercings. I wouldn't recommend the stretching immediately after a piercing thing, but large-gauge piercings (like 10G and up) are a great way to start if you don't have piercings already. Just make sure you wait about six months for them to heal/strengthen before you stretch them.

Beware of piercing/tattoo shops when it comes to stretching. A lot of them will tell you "Oh, you're fine to go up 4mm in one go!" or "you can stretch every week!" when that's just not true... you're much, much, MUCH better off doing it yourself and listening to the advice you get on this website and others (like bodyjewelleryshop.com).
