a blow out usually looks like a flap of skin or ridge of skin it looks similar to a gasket or a flesh colored o ring its usually on the back but can be on either side .. a blow out is actually wear you force the skin inside your fistula to turn inside out and then it ends up hanging out of the hole

here is an example of a pretty bad blow out that wasnt treated quick enough .

If you have any thought that it might be a blow out then downsize asap ! letting a blow out heal outside makes it really hard if not possible to correct it with out surgery ..
if it is a fresh blow out take some smaller plugs and put them in the opposite direction of the blow out to help tuck the blown out fistula skin back in and then just hope it heals into place .... good luck

is this considered blowout or swollen?

It doesn't look like a blowout yet but wearing a taper that size that's stainless steel can definitely cause one. Tapers aren't meant to be worn, they're just tools. Get a pair of single-flare or no-flare plugs. Actually you should downsize if you're having swelling after a stretch.
