What is this?

I just came across this image, & I've never seen this before. Being that it's not totally symmetrical, I was wondering if maybe she had her ears stretched normally, tore them, & then tried to do something with it. Or, since it looked like some stretching was involved, is this something people are doing now, stretching with the intent of eventually doing this? I don't even know where they must've gotten the jewelry, it must be a custom piece. It's definitely not a look for me, but hey, some think that regularly stretched ears looks ridiculous, so far be it from me to judge.
